Transmogrifier
Inspired by Calvin and Hobbes, Transmogrifier is a shop pool active item that gives Isaac 1/3 of a random transformation per use. With enough uses Isaac will obtain all of the transformations in the game. Type "giveitem Transmogrifier" in the mod console to give yourself the item or try the included Transmogrifier challenge.
Feel free to offer any suggestions to improve the mod or make it more fun. I’m open to adding transformations from other mods as well. To balance the item, I have made it require a variable number of charges depending on how many transformations Isaac possesses. Transmogrifier can take anywhere between 3-12 charges.
I give full permission to anyone who wants to take my mod, code, or graphics and do what they want with it.

Known Issues:
1) There is no way to tell if Isaac has already picked up an item and rerolled out of it. In that situation Transmogrifier would still give and remove an item, but the item has already been counted towards a transformation. Likewise if Transmogrifier says Isaac has 2/3s of given transformation, and then Isaac picks up one of the 2 items
2) SuperBum and Stompy are not listed as transformations in the PlayerForm enum so they are not included in this mod.
3) I have no clue how to display fortunes so I print text at the bottom of the screen letting the player know which transformation Isaac got closer to obtaining.
4) Balancing Transmogrifier will probably take some time.
