‘Help from the underworld’

When you pick up a black heart, a charmed imp is summoned to fight on your side! The imp stays with you until he dies. Spawns 2 black hearts on pick up.

This item can be very good with things that give black heart drops, like Daemon’s Tail for example.
Found in the Devil Room & Curse Room.
