This is a secret room item which will open a door to the error room of the floor, after the floor’s boss is killed, in a spot that’s free and also not red key compatible. If successful, this event is indicated by an obvious sound cue and -if you have MinimAPI installed- an icon on the minimap.
The door can spawn in any room besides ultra secret and boss rooms.
