Of the companion options available from Fiamma’s, change the “ little girl” option to one that allows you to select any NPC.
You can join an NPC by first performing a search and then selecting the NPC you wish to join from the list.
By default, the internal level of the NPCs you join is generated at the NPC’s base level, but you can generate a fixed level by setting the Level value in the config file to any value.
