Hold you context menu key and click "Valkyrie’s Menu" on the left side then click the "Gmod Death View" tab.

Commands:

cl_gdeathview [1/0] Default = ‘1’ [Enable/Disable the fps death view]
d_gdv_inf [used for sending debug/error reports]
