Official Black Death Scenario adapted to regular (sandbox) game.
Assets taken from scenario significantly reworked to fit sandbox game:
Walled District: Hospital, Mass Grave, Keep
Units: plague Doctor, Flagellant, Scholar
Policies: Quarantine Policy, Coerced Work, Studia Generalis
New Spy operation added: Spread plague

Main principle: disease start in cities with population more than 4 when at least one of the following conditions is satisfied:
a) city has foreign Trader stationed at the end of the turn;
b) city has negative housing;
c) city is starving (negative food).
City appeal, amenities, city buildings or districts have no effects on plague appearing probability.
Infected city will lose population 1 per turn.
Walled District, Hospital and Sever diminishes chances of population loss each turn by 15, 10 and 10 percents.
Quarantine policy adds additional 50%
So in total city chance to lose population could be as low as 15% (100 – 85 = 15)
Started within the city center plague spreads into adjacent tiles.
Probability of adjacent tile to get infected depends mostly if tile has road and is occupied (by district or improvement).
With Quarantine Policy Walled Quarter, Fort (improvement) and Military Land Unit on a tile drastically reduces chances tile get contaminated.
Also plague won’t spread into untouched Forest, Desert, Snow, Mountains tiles.
With all this you could effectively make quarantine "firewall" not allowing plague to spread further. And with Scholars or plague doctors to cure already infected tiles.
This of course requires advance planning, knowledge of the map and swift reaction to contamination.
This is intentionally made hard. Very hard.
In later eras however things get easier.
plague duration on the tile is related with World Era.
Most severe plague is during Medieval Era.
Later on with each era probability of a city to get plague goes down as well as duration of the plague gets shorter.
