Gaia – Playable character
While playing Gaia pickups have a lower spawn rate, luckily Gaia can create her own pickups by using the ‘essence’ of enemies she defeats.
Killing a bunch of monsters will cause her flower to glow, when it does you can press the SWAP button to choose from a list of pickups to spawn. (CTRL by default on keyboard)
The more enemies you kill the more options you have for what to create, including pills, cards, trinkets, items, even extra lives!
She also starts with "The World" card and the item "Book of Genesis", which is a 4 room active item that creates friendly copies of up to 6 non-boss enemies in the room, it can be found by other characters in angel rooms and libraries.
Should work with True Co-op, if ANY player is Gaia the pickup spawn rate will be lowered and all players share the flower charge for pickup spawning. (though only Gaia players will be able to spawn the pickups)
The pickups that are spawned when a room is entered or completed or when an enemy is killed have only a 1/3 chance of appearing, however items/cards/pills still have 100% the normal chance, chests have 2/3 chance and coins have 2/3 while in greed mode, secret rooms and error rooms are exempt from this and have 100% spawn rate for everything.
This also comes with ‘Curse of Scarcity’ in which most pickups have about 2/3 spawn chance, pickups spawned by gaia’s ability aren’t counted, unlike gaia’s natural spawn reduction it doesn’t matter when or how the pickup spawns, however the curse is pretty rare.
You can give yourself a challenge by using the console to give yourself the ‘Curse of Scarcity’ item which forces the curse, for a hardcore challenge there’s also the ‘Curse of Desolation’ item which stops pickup spawning completely (aside from items), you’ll have to rely entirely on the pickups Gaia spawns.

Tips:
Item spawning depends on the room you’re in, try using it in the devil deal room.
If a boss spawns minions try to farm the minions for a while instead of going for the boss.
Try to have enough charge to spawn a ‘special heart’ at the end of the floor and spawn one before moving to the next floor, you’ve got a decent chance of getting an eternal heart which then gives an HP up.
When starting a big fight activate the flower and pick heart, during the fight if you get hurt not only can you grab the health you spawned earlier but you can quickly get more health during invincibility frames by just double tapping CTRL.
If you get the D7 you can use it to ‘refresh’ rooms that had a large number of enemies in them for a bunch of extra essence.
