1.) Realism:
Tweaked settlement population requirements to take more people to go along with the 4tpy to make growth an actual mechanic, sometimes you need to sacrifice some income and stop training to grow a city now. In addition, troops now use and return a 1-1 value to population. So if you train 100 troops, it takes 100 population from the city, likewise if you disband 18 soldiers in a town you gain 18 population.

Tweaked travel distances to match a 4tpy system, and boosted ships to actually be valuable and worth the cost of maintaining, now you have to decide between a 5 turn march or a 1-2 turn ship ride. Don’t lose your fleets though or your armies will have a long ways home.

Very hard (battle difficulty) is punishingly fun if you are into that sort of thing!
