Cold Winters
Vanilla Zomboid climate of Kentucky is set up in such a way that winters are quite warm and mild. For instance, snow barely stays for more than a month between December and February, and it melts pretty often. Additionally, you may only encounter quite low temperatures a couple of times throughout the whole winter. Even if you change the temperature preset in Sandbox to "Cold" the temperatures will drop to -15 degrees for a few weeks in January. Actually, these temperatures are historically accurate if you check KY weather history in 90`s, but some players may find it not challenging enough.
The "Cold Winters" makes winter tougher: temperatures can drop to -30 degrees Celsius, and you’ll find yourself facing real cold by lowering winter temperatures, which is more challenging for your survival. Additionally, blizzards will occur more often (can be disabled in Sandbox settings), so be ready to cope with extreme weather. Moreover, there is a little chance in winter that your generator will failure due to cold and you have to restart it! You can disable it on world creation in sandbox settings.
Winter will turn into a season that requires serious preparation. It will also take some time in the beginning of spring to make plus temperature back. And for a better experience, I`d recommend you set the temperature preset to "Normal" if you play in sandbox. This will make the beginning of autumn/end of spring less cold.
